Applications are invited for a Senior Social Work Practitioner post in the East 2 Care and Support Team based in Cynon Principal Office, Trecynon Aberdare.

We are a fast paced team that works with individuals and their carers under the Social Services and Well Being Act (2014) to support service users and their families/carers with complex care and support needs to achieve positive personal outcomes. Therefore, the Senior Practitioner in this team is expected to be a proactive team player who can recognise and draw on the wealth of experience available to them within the Care and Support Teams. We are looking for a Senior Practitioner who has the experience and enthusiasm to play an active role in delivering a high quality service to our service users. A sound value base, excellent communication skills and a person-centred approach are essential. Knowledge of the Social Services & Well-Being Act, Mental Capacity Act, Best Interest process, Court of Protection work and Continuing Healthcare Assessments are fundamental to this role.

You will support the work of the Team Manager by taking on a coaching and mentoring role to promote good practice and team development. You will also have a supervisory role for some team members. You will be expected to provide expert advice and guidance to the team. You will carry a reduced caseload of the most complex cases and will need to work collaboratively with individuals and their families/carers to achieve positive outcomes; while balancing risk with individual independence and choice. In this role you will also assist the Team Manager with some of the administration and managerial tasks associated with the running of the team.

The successful candidate will have knowledge of the legislation underpinning Care and Support Services and will be required to demonstrate a commitment to continuous professional development.

You will need to hold a relevant professional Social Work qualification, i.e. a Degree in Social Work or recognised predecessor equivalent, must be registered with Social Care Wales as a ‘Social Worker’ and have three years post qualifying experience in a care management role.
