Adaptations and Community Equipment Team

Applications are invited for a qualified Community Occupational Therapist to join the Adaptations and Community Equipment Team based in Ty Elai.

As part of the wider Short Term Services, including Reablement, Support@Home services, Sensory Team and Short Term Care Management team, the ACE Team work closely with their colleagues to achieve positive outcomes for our service users and their families/carers. You will be joining an established and experienced team of Occupational Therapists and Community Care Workers who will be able to support you in making the most of this opportunity.

The Community Occupational Therapist in this team is expected to be a proactive team player who has the enthusiasm to play an active role in delivering a high quality service to our service users. A sound value base, excellent communication skills and a person-centred approach are essential.

The ACE team is part of a progressive and forward thinking service and one of its future aims is to be able to respond rapidly to the needs of vulnerable adults. The role of the Occupational Therapist will be integral to this and therefore candidates need to have a flexible outlook and be responsive to potential changes as this work develops.

Your principal duties will involve assessing individuals, as per the Social Services and Well Being Act (2014), recommendation of major and minor works of adaptations and identification and provision of equipment to support independence. You will also have the opportunity to use or develop manual handling skills.

Regular supervision will be provided by a Senior Occupational Therapist and training and development opportunities will be available during the period of employment.

You will need to be OT qualified and registered with the HCPC. Experience of the Disabled Facilities Grant process would be valuable but not essential as in house training will be provided. Some experience of functional assessment or community work would be an advantage but applications from newly qualified OT's would be welcome.

There is an expectation to take part in a rota to cover the Stay Well@home 2 response services 365 days a year.
