An exciting opportunity has arisen for a Care and Support Practitioner in Adult Services.

We want to ensure our services help people achieve their full potential. That means working with people to understand what matters to them and how we can help.

We are looking for practitioners who are positive and enthusiastic and can work with individuals, families, and carers in line with the requirements and principles of the Social Services and Wellbeing Act (Wales) 2014, and the Mental Capacity Act 2005. We are seeking practitioners who are able to put the person at the heart of their work.

The successful candidate will undertake assessments, draw up care plans with individuals and their carers, reviewing care and support needs. Working with individuals who live within challenging situations and complex needs, the ability to appropriately manage risk is essential. Carrying a varied case load, the successful candidate will need to have a focus on a recovery model, balance risk against self determination, and uphold the rights of the individual, with the ability to deliver quality services.    

Working as part of a team, but able to also work autonomously, the successful candidate will have excellent verbal and written skills and be computer literate. You will need to be organised, self-motivated and be able to prioritise competing demands on your time as well as being creative and resourceful and have a strong belief in empowering and enabling adults towards independence.

Professional development is encouraged and expected in RCT, and the Council has an excellent training department with varied opportunities to participate in training in line with the role of a care and support practitioner. Regular professional supervision is provided, and you will also have opportunity to become involved in peer group supervision.

It would be desirable to have previous experience of working in a care management role or social care within Adult Services, knowledge and understanding of working within adult health / social care and be conversant with all current legislation in Wales.

Adult Services is currently undergoing a remodelling. The existing vacancy is in our Long Term Care and Support team. You should be aware that, following the remodelling, you may be asked to work in a different team to the one that you initially join.
