We are seeking to appoint four skilled, enthusiastic and innovative people to join Rhondda Cynon Taf’s Independent Living Service.

One of the posts is based in the Cynon Principal Office, Aberdare with the other three based in Pentre, Rhondda.

The successful candidates will be required to provide one to one support to individuals with a range of mental health conditions and learning disabilities. The support will be outcome focussed in order to assist the person work towards achieving their personal outcomes and in line with the Code of Practice devised from the Social Services and Well- being (Wales) Act 2014.

Assuming key worker status to a designated number of individuals, you will devise Individual Plans to meet the needs/personal outcomes identified by the Assessment Teams. The focus of the service is to motivate and assist individuals to develop and maintain the skills necessary for independent living, to promote mental and physical well-being and to enable individuals to have a more inclusive life style within their communities.

You will need to be able to work on your own initiative as well as part of a team, which would include working with other agencies/partners.

Support will be delivered in the individuals own home and/or within a variety of community settings.

You will also be required to work flexibly in order to meet the needs of individuals that may include evenings and weekends.

As well as working on a one to one basis with individuals, candidates may also be required to facilitate group activities where relevant.

Strong communication skills are essential to the post and there is an expectation to work towards achieving a relevant Health and Social Care qualification at Level 3.
