An exciting opportunity has arisen to join the Single Point of Access Team as a Coordinator. The main base for the Single Point of Access is Ty Elai, however Coordinators work on rotation from Royal Glamorgan Hospital where they focus upon facilitating discharges from the site.

The Single Point of Access forms the core element of the Information, Advice and Assistance Service (IAA) as required by the Social Services and Wellbeing Act 2014. The Team undertake an assessment focusing on 'What Matters' to the individual and will seek to provide the most appropriate response to meet an individual’s needs.

The Single Point of Access plays a crucial role in facilitating hospital discharges, attending meetings with Health Colleagues, and ensuring appropriate support is in place for individuals leaving hospital.

The Single Point of Access is part of Adult Short Term Services and works closely with other teams to achieve positive outcomes for our service users and their families/carers.

We are looking for an individual who is enthusiastic and has experience of delivering a high quality service to the public. A sound value base, excellent communication skills and a person centred approach are essential.

The successful candidate will have knowledge of the legislation underpinning Community Care Services and will be required to demonstrate a commitment to continuous professional development. You will be required to either hold a Social Care qualification to level 4 (or working towards level 4) or above, possession of a recognised qualification in social work/therapies or have appropriate experience in a Community Care Field.

You will work on an out of hours rota covering the service from 8.30am – 8pm, 365 days a year.
