An opportunity has arisen for an individual to work alongside managers in the care and support service to support service change and development.

Some of the implications created by the SSWB Act are now building momentum and the key objective of this post is to support the developmental priorities across the service - specifically with regards to supporting:

People: We are committed to putting individuals and their needs at the centre of their care by giving them a stronger voice in both the design and delivery of our service.

Well-being: We are undertaking a cultural change programme to ensure our practitioners are supporting people to achieve what matters to them by building on their own strengths and resources.

Earlier intervention: We are developing our approaches to support independence and resilience through enhancing the carer’s assessments and Direct Payment offer and exploring new models of targeted support to minimise escalation of need.

Collaboration: we are collaborating with the GP clusters to test a new approach to working with primary care through opportunities presented by the transformation Fund.

A recognised professional qualification in Social Work such as a degree in Social Work or predecessor equivalent is essential for this post alongside a clear understanding of the Social Services and Wellbeing (Wales) Act 2014, the Health and Social Services agenda, and the context within which we are working.

The right individual will be solution focussed and motivated to ensure our service continually improves for people accessing support in Rhondda Cynon Taf.

Support and equipment will be available for the post-holder to work from home with access to the office in Ty Elai as required.
