An opportunity has arisen in the Support @ Home (Intermediate Care and Reablement) service for a permanent Occupational Therapist.

You will become part of the Support @ Home Team. The team is based in Ty Elai and provides service users with an opportunity to recover from ill health and prevent episodes of health deterioration where possible.

The Reablement team offers service users a period of rehabilitation to enable them to maintain, regain or relearn basic living skills, providing support to enable people to reach their full potential whilst preventing or reducing the need to move onto longer-term community services.

The team supports individuals living in the community to become as independent as possible using programmes of up to 6 weeks duration.

The service has the benefits of a range of professional support, including Care Staff, supervisors, administrators, Occupational Therapists, Physiotherapists, an Occupational Therapy Assistant (OTA), and Assistant Therapy Practitioners (ATP). The team is supported by a reablement Team Lead.

You will be a qualified OT who has experience of providing Clinical supervision for staff or students. Ideally you will have proven experience in a community setting, however consideration will be given to candidates who have had community experience as part of their training or work experience. You will carry a case load and provide supervision and support for allocated members of staff, you will in turn receive supervision and support from the Team Lead. Full support, training and CPD opportunities will be provided for the successful candidate to develop skills and competencies and to assist with implementing changes to practice and service delivery.

There will be some occasional requirements to work evenings and Weekends, including bank holidays.
