Senior Practitioner - Cwm Taf Stay Well @ Home Team

This post is 33.6 hours per week working a 12 hr shift pattern - the salary and leave is pro rata. The post is based in Royal Glamorgan Hospital but there will be an expectation that the post holder may have to travel to Prince Charles Hospital if the service required this.

An exciting opportunity has arisen to join the National Award winning Stay Well @ Home hospital based team as the Senior Practitioner.

This approach to working in partnership has transformed the experience of people in hospital, particularly at A&E, and through a revised approach to early intervention, joint working and flexible deployment of resources ensures people are able to return to their home earlier with support rather than face prolonged admission to hospital.

The Stay Well @ Home Team consists of a multidisciplinary hospital based team (Social Workers, Occupational Therapists, Physiotherapists and Therapy Technicians), sited within the acute hospitals of Prince Charles (PCH) and Royal Glamorgan (RGH). The service operates 7 days a week (including Bank Holidays) between the hours of 8am and 8pm and is supported by a range of community based responses across health, social care and third sector provision.

The Senior Practitioner will be responsible for:-

  • Supporting the Team Manager to ensure the efficient and effective working of the hospital based team.
  • Ensuring a timely, professional and appropriate integrated assessment and response service, that prevents unnecessary admission and supports the default position that individuals are supported to return to a community setting.
  • Assessing patients/service users and applying innovative approaches and strength based solutions to prevent, or reduce the likelihood of hospital admissions.
  • Ensuring effective communication between health and social care colleagues to ensure that individuals have every opportunity to remain within their own home by ensuring safe, appropriate and effective hospital discharge arrangements are in place.

The experience and skills necessary for the Stay Well @ Home Senior Practitioner post will be:

  • Possession of a recognised qualification in Social Work.
  • At least three years' post-qualifying experience in a Social and/or Health Care setting.
  • Wide ranging knowledge of the legislative framework pertinent to Social Care and NHS functions.  
  • Experience of dealing with complex individual cases and/or situations.
  • Experience of working in a multi disciplinary/agency way.
  • Experience of meeting deadlines in a busy environment.

The personal qualities necessary for the Stay Well @ Home Senior Practitioner post will be;-

  • A self starter, able to motivate self and others to achieve goals.
  • Assertive and willing to challenge appropriately.
  • Proactive, creative and innovative.
  • Organised and able to prioritise.
