An exciting opportunity has arisen to join the Community Health and Wellbeing teams leading the development of the enhanced community clusters in collaboration with Cwm Taf Morgannwg University Health Board. The posts are being developed following a successful bid to Welsh Government for Transformation Grant monies, building on various innovative schemes such as the ‘virtual ward’ model developed in St Johns’ GP practice in the Cynon Valley. We currently have two vacancies covering the Rhondda and Cynon clusters.

Health and Social Care services across the country are facing a period of unprecedented pressure, and services need to change to meet this demand. RCT are keen to explore are the benefits of closer integration of all community-based services and the opportunities this will present in terms of anticipatory care, contingency plans and preventative services and support.

The work has an emphasis on multi-agency working and you will be part of a multi-agency team as well as having close links with the police and housing. You will be expected to understand and work within Safeguarding policies and the Mental Capacity Act 2005. An understanding of Continuing Health Care and Care and Treatment planning under the Mental Health Measure (Wales) 2010 would be helpful. We work to a preventative, strength-based model under the principles of the Social Services and Wellbeing Act 2014.

The successful candidates will need to have good verbal and written skills and have basic computer knowledge. You will need to be organised, self-motivated and able to prioritise competing job demands as well as being creative and have a strong belief in empowering adults towards independence.

The appointed social workers will be line managed and supervised via the long-term Care and Support teams. You will work to a preventative, strength-based model under the principles of the Social Services and Wellbeing (Wales) Act 2014 and will work in partnership with primary care and community clinicians in multi-disciplinary ‘clusters’, ensuring as much care is provided as close to home as possible.

Professional development is encouraged and expected, and the Council has an excellent training department with varied opportunities to participate in post qualifying accredited training. There is regular professional supervision provided on a six weekly basis and you will also have opportunity to become involved in peer group supervision. The teams are highly supportive environments to work in and RCT also offers the opportunity to maintain a good work life balance with staff benefits available to the workforce.

You must hold a recognised Social Work qualification and be registered as a 'Social Worker' with Social Care Wales on appointment.
